A referral is required for Medicaid billing. Please reach out to your physician and ask for a referral to be faxed to (806)216-6707.

Please note that the best practice is for you to reach out to your insurance and verify coverage for medical nutrition therapy. Most plans will provide preventative and medical coverage in some amount.

We work with you and your insurance company to get our services covered but any uncovered bills are the patient’s responsibility.

Medicare will only cover for diabetes and chronic kidney disease - if you do not have either of these diagnoses, contact us for out of pocket pricing.
