Request a Referral

If you have an HMO plan or Texas Medicaid, you will need a referral and/or a prior authorization from you PCP in order to have insurance coverage. You can reach out to your PCP office, or we can do it for you! We will call you to schedule an appointment once we have received the information needed from your insurance.
